目的

探讨新生儿Ⅲ型先天性食管闭锁围手术期的护理措施及效果，以为临床护理工作提供借鉴。

方法

回顾性分析新疆维吾尔自治区人民医院小儿外科，自2008年7月至2016年7月收治的56例Ⅲ型先天性食管闭锁新生患儿的病案资料，对比分析常规外科护理(对照组)及专科围手术期护理(观察组)方法的效果。

结果

56例患儿手术均成功，观察组术后并发症发生率低于对照组；观察组的住院时间12～21 d(14.26±3.27)，体重增加量为15～24 g(18.00±8.23)，与对照组比较差异有统计学意义(均P<0.01)。

结论

在常规儿外科护理干预基础上应用专科针对性的围术期护理取得理想的效果，值得在临床推广应用。

Objective

to study the perioperative nursing measures and effects of congenital esophageal atresia(type III)in neonates.

Methods

a retrospective study was conducted on 56 cases of congenital esophageal atresia in our department of pediatric surgery from July 2008 to July 2016.28 cases of routine surgical nursing intervention(n=28), 28 patients(n=28)were treated with conventional surgical nursing intervention based on perioperative care.Perioperative intervention: The preoperative nursing care for children with oral cavity(vomit white foam secretion into milk or water with cough and vomiting after facial bruising, tube or pipe can not be placed in difficulties in children with gastric contents through reflux into the trachea esophageal fistula caused respiratory tract obstruction, sputum aspiration of gastric contents were visible, airway care and environmental temperature nursing, intravenous nutrition care, family management etc). Intraoperative Nursing(anesthesia, nursing cooperation, operation safety check, intraoperative warming measures, surgical safety visits, intraoperative care of each channel, vital signs monitoring, etc). Postoperative nursing(monitoring of vital signs, take measures to prevent the occurrence of hypothermia scleredema.

Results

56 patients were successful operation, the complication rate is lower than the control group after the surgery of the observation group; the observation group in time of 12~21 days(14.26+ 3.27)days, the increase of body weight was 15~24 g(18+ 8.23)g, and the control group are different statistical significance(P<0.01).

Conclusion

on the basis of nursing intervention in routine pediatric surgery, the targeted perioperative nursing effect is statistically different, and it is worth popularizing in clinical practice.
