重新认识Toupet部分胃底折叠的临床价值

根据胃食管反流病"双括约肌"抗反流屏障原理,重建抗反流屏障包括修补加强食管裂孔和构建胃底折叠圈两个步骤。Nissen观察到食管内翻缝合的方法有利于降低胃食管反流的发生,而提出了经典的360°Nissen胃底折叠术。为了降低完全折叠后出现的吞咽困难,部分折叠的术式应运而生。针对部分折叠是否存在抗反流屏障过弱,以及是否应根据患者食管动力结果选择折叠方式的问题一直存在争议,而且Toupet在早期荟萃分析中所占比例也偏低。但随着近期高质量文献的发表,我们应当重新认识以Toupet为代表的部分折叠术式的临床价值。
