目的

通过分析支气管哮喘合并胃食管反流病女性患者的支气管激发试验、肺功能、食管24 h pH监测特点,探讨不同类型的食管酸暴露对女性支气管哮喘患者气道反应性及肺功能的影响。

方法

回顾性分析2015年1月至2017年6月,新疆维吾尔自治区人民医院呼吸科住院并怀疑合并GERD的36例女性支气管哮喘患者的食管24 h pH监测、支气管激发试验以及肺功能测试等临床资料,根据食管24 h pH监测中DeMeester评分判断是否存在病理性酸反流将样本分为GERD组(15例)及非GERD组(21例),分析其食管酸暴露特点与气道反应性及肺功能的联系。

结果

支气管激发试验Dmin值比较中,GERD组略高于非GERD组,但结果并无统计学意义(P>0.05);FEV1占预计值百分比(FEV1%)、FEV1/FVC、FVC占预计值百分比(FVC%)、RV/TLC方面2组均无统计学差异(P>0.05);GERD组患者食管24 h pH监测中pH≤4(酸反流)的总次数较非GERD组更多,差异有统计学意义(P<0.05);4<pH<7(弱酸反流)总事件数、食管近端总反流百分比、食管近端弱酸反流百分比在非GERD组中略高,但与GERD组之间并无统计学差异(P>0.05);GERD组的总反流事件、立位总反流事件、卧位总反流事件与非GERD组亦无统计学差异(P>0.05)。

结论

以pH≤4的酸反流事件增多为代表的病理性酸反流不能使女性支气管哮喘患者的气道反应性增高,亦不能引起其肺功能的显著改变,因该研究样本量低,仍需多中心、大样本研究来证实此结论。

Objective

To explore the corelation between different types of esophageal exposure and airway responsiveness and pulmonary funtion through 24-hour ambulatory pH monitoring, bronchial provocation test and pulmonary function test on asthmatic females.

Methods

All selected 36 female asmatic patients were devided into 2 groups based on whether their DeMeester scores in pH monitoring tests were higher than 14.72 or not.Analyse the Dmin score, pulmonary function and the type and severity of acid exposure.

Results

The Dmin score, FEV1% predicted, FEV1/FVC, FVC% predicted, RV/TLC showed no statistical difference between 15-sample GERD group and 21-sample Non Gerd group(P>0.05), nor did times of weak acidic(4<pH<7)reflux, total reflux episodes, supine or upright relux episodes, percentage of proximal reflux(P>0.05). But the acidic(pH≤4)reflux events in patients with GERD ware greater than those without GERD(P<0.05).

Conclusion

Our findings suggest that GERD, especcially associated with typically acidic reflux, does not aggravate the status of airway hyperresponsiveness or the pulmonary function in asmatic females.
