目的

探讨延续性护理干预对减重代谢术后改善患者胃食管反流病（GERD）症状、生活质量及减重效果的疗效。

方法

回顾性分析2020年1月至2022年11月在新疆维吾尔自治区人民医院接受减重代谢手术的65例肥胖患者的临床资料，采用数字表法随机分为对照组（31例）和观察组（34例）。对照组患者给予常规护理方法（住院期间-出院后来院复查时进行）；研究组患者在对照组基础上联合给予延续性护理干预治疗（住院期间-出院后-居家链式管理为期6个月）。2组患者在治疗前后均收集并分析腰围、臀围、血糖、血压、GERD症状评分表和健康促进生活方式量表（HPLP-Ⅱ）的变化差异。

结果

2组术后随访6个月的体质量、体质量指数（BMI）、腰围、臀围、额外体质量减轻百分比（%EWL）、空腹血糖（FPG）、血压、GERD评分和HPLP-Ⅱ评分等与术前比较差异有统计学意义（P<0.05）。与对照组相比，观察组的体质量、BMI、腰围、臀围以及%EWL等指标的差异有统计学意义（P<0.05）。与对照组相比，观察组的GERD评分和HPLP-Ⅱ评分的差异有统计学意义（P<0.05）。观察组的总有效率显著高于对照组，差异有统计学意义（97.06%比90.32%，P<0.05）。

结论

减重术后延续性护理干预的长期减重效果较好，有效改善患者GERD症状的发生并提高患者生活质量。

Objective

To explore the efficacy of continuation nursing intervention on the improvement of gastroesophageal reflux disease (GERD) symptoms, quality of life and weight loss after weight loss metabolism.

Methods

Retrospective analysis was made on 65 obese patients who received weight loss and metabolic surgery in the People's Hospital of Xinjiang from January 2020 to November 2022. They were randomly divided into the control group (31 cases) and the observation group (34 cases) by using the digital table method. The control group patients were given routine nursing methods (during hospitalization - during hospital follow-up after discharge); The study group patients were given continuous care intervention treatment in combination with the control group (during hospitalization, after discharge, and at home chain management for a period of 6 months). Both groups of patients collected and analyzed changes in waist circumference, hip circumference, blood sugar, blood pressure, GERD symptom score, and health promoting lifestyle scale (HPLP - II) before and after treatment.

Results

The body weight, body mass index (BMI), waist circumference, hip circumference, percentage of extra weight loss (%EWL), fasting blood sugar (FPG), blood pressure, GERD score and HPLP - Ⅱ score in the two groups were significantly different from those before surgery in 6 months of follow-up (P<0.05). Compared with the control group, the differences in indicators such as body weight, BMI, waist circumference, hip circumference, and %EWL in the observation group were statistically significant (P<0.05). Compared with the control group, the difference in GERD score and HPLP - II score between the observation group was statistically significant (P<0.05). Compared with the control group, the observed total effective rate was significantly higher (97.06% vs 90.32%, P<0.05).

Conclusion

The long-term weight loss effect of continuous nursing intervention after weight loss surgery is good, effectively improving the occurrence of GERD symptoms in patients and improving their quality of life.
