食管裂孔疝(HH)是外科常见病。在临床上主要分为4型,Ⅰ型滑动型食管裂孔疝和Ⅱ~Ⅳ型食管旁疝。目前外科治疗以食管裂孔疝修补术结合抗反流手术为主,且对于在术中使用补片加固形成了较统一的共识,但术式选择、补片材料选择等仍具有争议。高龄和肥胖是HH的高危因素,年龄的精细分级和手术选择依旧是我们探索的方向。对于肥胖合HH的患者,应常规行HH修补术或胃底折叠术。笔者将通过对食管裂孔疝外科治疗中的一些困惑做分析总结,以期对食管裂孔疝的研究提出新思路。

Hiatal hernia (HH) is a common surgical disease.Clinically, it is mainly divided into type 4: type I is sliding esophageal hiatal hernia and type Ⅱ-Ⅳ are paraesophageal hernia. At present, the surgical treatment is mainly for esophageal hiatal hernia repair combined with anti-reflux surgery, but the surgical choice and mesh choice are still controversial. Elderly patients and obesity are high risk factors for HH, and the grade of age and surgical choice are still the direction of our exploration. For obese patients with HH, HH repair or fundoplication should be performed routinely. The author will analyze and summarize some puzzles in the surgical treatment of hiatal hernia,In order to propose new ideas for the study of esophageal hiatal hernia.
