目的

评估唾液胃蛋白酶检测对反流性咽喉炎(LPR)患者的诊断价值。

方法

选择2018年1月至2018年6月就诊于新疆自治区人民医院耳鼻咽喉科门诊30例LPR患者作为试验组,反流症状指数量表(RSI)>13分,反流体征指数量表(RFS)>7分;50名健康成年人作为对照组。收取受试者晨起空腹第一口唾液及睡前最后一口唾液,封存于1.5 ml无菌EP管内,并在30 min内将EP管放置冰箱中冷冻,采用人胃蛋白酶ELISA检测试剂盒测定受试者唾液中胃蛋白酶含量。

结果

试验组晨起唾液中胃蛋白酶含量为122.964(49.224~239.004)ng/ml;夜间唾液中胃蛋白酶含量为98.116(51.116~143.792)ng/ml。对照组晨起唾液胃蛋白酶含量为14.57(2.62~32.04)ng/ml;夜间唾液中胃蛋白酶含量为14.22(7.72~20.3)ng/ml,经过Mann-Whitney U检验,差异均有统计学意义(P<0.01)。

结论

胃蛋白酶试纸条检测有无创、价廉和易于操作的优点,作为一种初期诊断咽喉反流的客观方法是可行的,值得临床推广应用。

Objective

To determine whether detection of pepsin in the saliva could be useful for diagnosing laryngopharyngeal reflux (LPR).

Methods

30 patients having LPR with reflux symptoms index scale (RSI)>13 and a reflux symptoms index scale (RFS)>7 were selected. 50 healthy adults were randomly recruited as subjects,and the saliva was collected into 1.5ml sterile EP tubes after awaking in the morning,and the last saliva in the evening.and the tubes were placed in a refrigerator with in half an hour. The level of pepsin in saliva was measured by human pepsin ELISA kit.

Results

Among the 30 subjects,the average content of pepsin in morning saliva was 122.964 (49.224-239.004) ng/ml. The average content of pepsin in evening saliva was 98.116 (51.116-143.792) ng/ml. Among the 50 subjects, the average content of pepsin in morning saliva was 14.57 (2.62-32.04) ng/ml. The average content of pepsin in evening saliva was14.22 (7.72-20.3) ng/ml. There were statistical differences between the laryngopharyngeal reflux group and healthy group in the saliva and pepsin level (P<0.01).

Conclusion

Pepsin strip detection has the advantages of non-invasive,cheap and easy to operate. As an objective method for early diagnosis of laryngopharyngeal reflux, pepsin strip detectionis feasible Which should be popularized.
