目的

探讨内镜下贲门缩窄术（PECC）治疗胃食管反流病（GERD）对患者生活质量的改善情况。

方法

收集2019年2月至2021年4月于新昌县人民医院接受PECC治疗的GERD患者59例的病例资料。对比分析治疗前及治疗后1、2、3、6个月患者的反流症状评分（RDQ）和胃肠道生活质量指数（GIQLI）、患者满意度、质子泵抑制剂（PPI）使用情况及患者的术后并发症情况。

结果

59例GERD患者均顺利完成PECC治疗，术后随访1、2、3、6个月，患者反流症状及生活质量均明显好转。RDQ积分自治疗前的（15.4±2.3）分降至治疗后1个月的（6.1±2.8）分，治疗后2个月的（4.4±1.4）分，治疗后3个月的（4.2±1.2）分，治疗后6个月的（4.0±1.0）分，差异均有统计学意义（P<0.05）。GIQLI积分自治疗前的（73.8±13.6）分升至治疗后1个月的（102.8±12.0）分，治疗后2个月的（117.2±10.9）分，治疗后3个月的（122.6±7.6）分，治疗后6个月的（122.9±7.0）分，差异均有统计学意义（P<0.05）。PECC术后1个月，28例（47.5%）患者完全无需服用PPI；术后2个月，41例（71.9%）患者完全无需服用PPI；术后3、6个月，均有47例（92.2%）患者完全无需服用PPI，术后患者PPI使用量较术前明显减少，治疗满意度较高。

结论

PECC能够改善GERD患者的生活质量，减少PPI使用。

Objective

To study the survey on the quality of life of patients with gastroesophageal reflux disease undergoing peroral endoscopic cardial constriction.

Methods

In this study, 59 patients with gastroesophageal reflux disease (GERD) who visited the People's Hospital of Xinchang from February 2019 to April 2021 and received peroral endoscopic cardial constriction (PECC) were included. The reflux diagnostic questionnaire (RDQ), gastrointestinal quality of life index (GIQLI), satisfaction rate, proton pump inhibitor(PPI) usage and postoperative complications were compared and analyzed before and 1, 2, 3, 6 months after treatment.

Results

All of the GERD patients received PECC successively and no severe adverse events or complications occurred. The RDQ score decreased from 15.4 ± 2.3 before treatment to 6.1± 2.8, 4.4±1.4, 4.2±1.2 and 4.0±1.0 after treatment for 1month,2months,3months and 6months, with statistically significant differences (P<0.05). The GIQLI score increased from 73.8±13.6 before treatment to 102.8±12.0, 117.2±10.9, 122.6±7.6, 122.9±7.0 after treatment for 1month,2months,3months and 6months, with statistically significant differences (P<0.05). One month after PECC, 28 patients (47.5%) did not need to take PPI at all; two months after operation, 41 patients (71.9%) did not need to take PPI; 47 patients (92.2%) did not need to take PPI afterr PECC 3 to 6 months. The amount of PPI usage after PECC was significantly reduced and the treatment satisfaction was high.

Conclusion

PECC can improve the quality of life of GERD patients and reduce the amount of PPI usage.
