肥胖是引起胃食管反流病(GERD)发生发展的独立危险因素之一,肥胖和GERD的发病率近年来同步增长,而且肥胖患者绝大多数合并GERD。腹腔镜胃袖状切除术(LSG)和腹腔镜Roux-en-Y胃旁路术(RYGB)是目前国内外最常用的减重术式。然而,两者在治疗合并GERD的肥胖患者长期抗返流疗效方面存在争议。腹腔镜胃底折叠术联合胃袖状切除术(LFDSG)是一种新型手术技术,能将胃底折叠术的抗反流作用和胃袖状切除术的减重作用联合在一起,以达到两种手术扬长避短的目的,有望为临床上治疗肥胖合并GERD或食管裂孔疝的患者提供新型抗反流减重手术。本文总结现有的主要的减重术式(LSG、RYBG)以及新型抗反流减重(LFDSG)手术在治疗肥胖合并GERD的疗效方面的研究进展以及工作经验,为外科医生提供借鉴。

Obesity has become one of the most serious diseases that affecting peoples health in worldwide. Bariatric surgery nowadays thought one of the most curative method for obesity and related complications. Obesity also concidered to a important risk factor for occurrence of gastroesophageal reflux disease (GERD). The incidence of obesity and GERD is increasing in a parallel way, and the majority of those obese patients also suffering GERD. Laparoscopic sleeve gastrectomy (LSG) and laparoscopic Roux-en-Y gastric bypass (RYGB) are the most accepted methods in world wide . However, the efficacy of the two methods in treating patients with obesity and GERD is controversial. Laparoscopic fundoplication with sleeve gastrectomy (LFDSG) is a new surgical technique. It may combine the anti-reflux effect of fundoplication with the weight-loss effect of sleeve gastrectomy in order to achieve the purpose of enhancing the advantages and avoiding the disadvantages of the two operations. LFDSG hopefully could provide a new anti-reflux and weight-loss for the treatment of obesity with GERD or hiatal hernia. This article summarizes the research progress and experience of the main weight-loss methods (LSG, RYBG) and the new anti-reflux weight-loss (LFDSG) in the treatment of patients whom with obesity and GERD, and provides a nes insight for surgeons.
